Santa Clara Reentry Services (AB109)
We reintroduce non-serious felons released from prison back into the community by providing dual diagnosis treatment.
Adult Outpatient Ethnic Specific
Our culturally competent mental health services for Asian Pacific adults and older adults provides counseling that fit the cultural needs and to improve life and restore relationships. Program is for those with serious mental illness, or co-occurring substance abuse and mental illness.
Adult Program
Through accurate diagnosis and appropriate treatment plans we help individuals between the ages of 18 and 59 years of age from being prematurely institutionalized.
California Department of Correction and Rehabilitation
Our program provides intensive outpatient services to individuals in the Adult Criminal Justice Program struggling with dual diagnosis issues. Our services are designed to prevent recidivism, mental health and substance abuse relapse. Services include holistic treatment via counseling, case management, mediation, exercise, and nutrition.
CalWORKS
We provide intensive services to families receiving Temporary Assistance to Needy Families (TANF) support. Our program addresses domestic violence, mental health, substance abuse and related issues. We also support families in their readiness for employment and self-sustainability.
Cambodian Culturally Specific Family Services
We offer family-focused mental health services to Cambodian Genocide Survivors and low income Cambodians residing in Santa Clara County. Services may include counseling, crisis intervention, medication support, case management and/or mindfulness meditation groups.
Differential Response Case Management
We teach families to use resources in their community and help them with parent coaching and education. Families may be referred to our Differential Response Therapeutic Services program listed below.

Ethnic & Culture Specific Wellness
The Ethnic and Culture Specific Wellness Program at Gardner Health Services aims to build awareness about mental health and related services and resources for the Latino and African descent communities of the Santa Clara County. The program includes outreach activities in the heart of the communities and from our centers at various locations in the county including San Jose, Alviso, and Gilroy. We hope to create awareness, decrease cultural bias and stigma about mental health challenges and participate in advancing culturally sensitive treatment and care to improve the wellbeing of our community members.
Family Strength Based Services
Our counselors provide services to families to reduce the risk of abuse and prevent further involvement in the foster care system. Services include counseling, parenting support and community referrals.
Full Service Partnership – Adult & Older Adult Programs
Our Full Services programs provide comprehensive treatment and management for those who have a severe mental illness and need intensive services. Full Service Partnerships are based on the idea of doing “whatever it takes” to help people on their path to wellness and recovery. Full Service Partnerships programs help with jobs, school and temporary housing. Services can be provided in the home and the community.
Full Service Partnership – Criminal Justice Program
This program delivers individualized recovery/treatment plans containing measurable treatment goals. Our clients receive intensive case management, individual and family treatment, psycho-educational and therapeutic process groups, psychiatry, and peer mentorship.
Older Adult Prevention & Early Intervention
Counseling is provided to older adults who may benefit from behavioral health services but have not received any consistent behavioral health services over the previous year. Providing early treatment that will prevent the individuals from needing more extensive services. The staff provide mental health counseling, anger management and socialization programs. There is an emphasis on working with home-bound adults with signs of isolation, anxiety, depression, intra-familial discord, general neglect, and substance abuse.
Post Release Community Supervision Program
Offering one of only six programs funded by the State of California to provide outpatient treatment for newly released prisoners. Our therapists assists in connecting clients to resources essential to their recovery, wellness and long term success.
